JAGLITES - PRODUCT RETURN FORM
If you wish to return an item please follow the steps below:

- Send an e-mail to returns@jaglites.com and obtain an RMA# (Return Authorization Number).
- Include this completed form and a copy of the packing slip with your return.
- Write in bold print either on the shipping label or outside of the box the RMA# (Return Authorization Number).

- Original freight charges are not refundable.
- Returns will be accepted for items returned with a RMA # up to 30 days from the date of invoices.
- Customers are responsible for return shipping of the product to our warehouse. In order to track your return,
please ship via UPS, FedEx or other shipping company that provides tracking.
- All returns must be new, unused and in their original packaging including all original hardware, instructions and materials.
Returned items must be in a re-saleable condition. Any items showing wear, soiling, damage, modification, or signs
of being installed will not eligible for credit.

Any package received without a Return Authorization Number (RMA) on the outside of the box or
returned 30 days after the invoice date will be refused. JAG Lites Corp. will not
issue credit on any equipment that has been installed, used or returned in a non re-saleable condition.
